Cyclooxygenase 2 (COX-2) is a bifunctional enzyme with both cyclooxygenase and peroxidase activities, primarily involved in the biosynthesis of prostanoids—a class of C20 oxylipins derived mainly from arachidonate—playing a pivotal role in inflammatory responses. Its cyclooxygenase activity oxygenates arachidonate (AA, C20:4(n-6)) to prostaglandin G2 (PGG2), while the peroxidase activity reduces PGG2 to prostaglandin H2 (PGH2), the precursor of all 2-series prostaglandins and thromboxanes. This transformation involves hydrogen abstraction at carbon 13 (S-stereochemistry), followed by molecular oxygen insertion to form the endoperoxide bridge between carbons 9 and 11 (defining prostaglandins), and a second oxygen insertion (bis-oxygenase activity) to yield PGG2, which is then reduced to PGH2. Similarly, COX-2 catalyzes the sequential cyclooxygenation and peroxidation of dihomo-γ-linolenate (DGLA, C20:3(n-6)) and eicosapentaenoate (EPA, C20:5(n-3)) to produce PGH1 and PGH3 (precursors of 1- and 3-series prostanoids) (By similarity). In an alternative pathway, it converts 2-arachidonoyl lysophospholipids to prostanoid lysophospholipids, hydrolyzed later to free prostanoids (By similarity). COX-2 also metabolizes 2-arachidonoyl glycerol to glyceryl PGH2 (potentially contributing to pain) (By similarity) and generates lipid mediators from n-3/n-6 PUFAs via a lipoxygenase-like mechanism, oxidizing PUFAs to hydroperoxides before reducing them to alcohols (By similarity). During inflammation resolution, COX-2 produces resolvins: converting DHA (C22:6(n-3)) to 17R-HDHA (precursor of D-series resolvins); in E-series resolvin synthesis, primarily transforming EPA to 18S-HEPE (further processed to 18S-RvE1/2 by ALOX5/LTA4H); and in endothelial cells, oxidizing DPA (C22:5(n-3)) to 13R-HDPA (activates macrophage phagocytosis) (By similarity). In activated leukocytes, it oxygenates HETEs to diHETEs (e.g., 5,15-diHETE) (By similarity) and stereospecifically converts linoleate (LA, C18:2(n-6)) to (9R)-HODE and (13S)-HODE (By similarity). During neuroinflammation, COX-2 facilitates neuronal secretion of 15R-lipoxin A4, regulating microglial phagocytosis (PubMed:29662056).
