CaMKII delta is a calcium/calmodulin-dependent protein kinase that regulates Ca²⁺ homeostasis and excitation-contraction coupling (ECC) in the heart by targeting ion channels, transporters, and accessory proteins involved in Ca²⁺ influx, sarcoplasmic reticulum (SR) Ca²⁺ release, SR Ca²⁺ uptake, and Na(+)/K(+) channel transport. It also modulates cardiac function by targeting transcription factors and signaling molecules. In its activated state, CaMKII delta contributes to the pathogenesis of dilated cardiomyopathy and heart failure by directly phosphorylating RYR2 Ca²⁺ channel at 'Ser-2808', thereby regulating SR Ca²⁺ release and promoting cardiac decompensation. In the nucleus, it phosphorylates the MEF2 repressor HDAC4, triggering its nuclear export and binding to 14-3-3 protein, which leads to MEF2 activation and expression of hypertrophy-related genes. This kinase is essential for left ventricular remodeling post-myocardial infarction and acts downstream of β-adrenergic receptor signaling to regulate key ECC proteins. It modulates Ca²⁺ influx by binding and phosphorylating the L-type Ca²⁺ channel subunit beta-2 CACNB2. Additionally, CaMKII delta targets and regulates the cardiac sarcolemmal Na(+) channel Nav1.5/SCN5A and the K(+) channel Kv4.3/KCND3, both implicated in arrhythmogenesis during heart failure. By phosphorylating phospholamban (PLN/PLB), an endogenous inhibitor of SERCA2A/ATP2A2, it enhances SR Ca²⁺ uptake, which may be critical for frequency-dependent acceleration of relaxation (FDAR) and maintaining contractile function during acidosis. It may also participate in exercise-induced skeletal muscle adaptation by regulating SR Ca²⁺ transport via phosphorylation of PLN/PLB and triadin, a ryanodine receptor-coupling factor. Under interferon-gamma (IFN-γ) stimulation, CaMKII delta phosphorylates STAT1, activating the JAK-STAT pathway (by similarity).
