Background

IL6 is a cytokine with diverse biological roles in immunity, tissue regeneration, and metabolism. It binds to IL6R, forming a complex with the signaling subunit IL6ST/gp130 to activate the intracellular IL6-signaling pathway (Probable). Interaction with membrane-bound IL6R and IL6ST triggers 'classic signaling,' while binding of IL6 and soluble IL6R to IL6ST initiates 'trans-signaling.' Alternatively, 'cluster signaling' occurs when membrane-bound IL6:IL6R complexes on transmitter cells activate IL6ST receptors on adjacent receiver cells (Probable). IL6 is a potent inducer of the acute phase response, with rapid production aiding host defense during infection or injury, though excessive synthesis contributes to disease pathology. In innate immunity, IL6 is produced by myeloid cells (e.g., macrophages, dendritic cells) upon pathogen recognition via TLRs at infection or injury sites (Probable). In adaptive immunity, it is essential for B-cell differentiation into antibody-secreting cells and plays a key role in CD4(+) T-cell subset differentiation, particularly in T follicular helper (Tfh) cell development for germinal-center formation and Th17 lineage commitment. It also supports myeloma cell proliferation and plasmablast survival (By similarity). IL6 is critical for bone homeostasis and vascular function, directly or indirectly promoting angiogenesis and vascular permeability via VEGF induction (PubMed:12794819, PubMed:17075861). Synergistically with IL1B and TNF, it drives VEGF production through 'trans-signaling' (PubMed:12794819). As a metabolic regulator, IL6 released post-muscle contraction enhances lipolysis and insulin sensitivity (PubMed:20823453). Central nervous system 'trans-signaling' modulates energy and glucose homeostasis (By similarity). Via GLP-1, it coordinates crosstalk among insulin-sensitive tissues, gut L cells, and pancreatic islets to adapt to insulin demand (By similarity). It acts as a myokine (Probable), protects during liver injury by supporting regeneration (By similarity), and regulates iron metabolism via HAMP/hepcidin expression during inflammation or infection (PubMed:15124018). Through IL6ST-YAP-NOTCH activation, it promotes inflammation-driven epithelial repair (By similarity).
