Background

RAGE is a cell surface pattern recognition receptor that senses diverse endogenous stress signals with a broad ligand repertoire, including advanced glycation end products (AGEs), S100 proteins, high-mobility group box 1 protein (HMGB1), amyloid beta (APP) oligomers, nucleic acids, histones, phospholipids, and glycosaminoglycans. These ligands accumulate at inflammatory sites during the pathogenesis of various diseases such as diabetes, vascular complications, neurodegenerative disorders, and cancers, where RAGE transduces their binding into pro-inflammatory responses. Upon ligand binding, RAGE utilizes TIRAP and MYD88 as adapters to activate NF-kappa-B, leading to the induction of inflammatory cytokines IL6, IL8, and TNFalpha. Interaction with S100A12 triggers cellular activation in endothelial cells, mononuclear phagocytes, and lymphocytes, generating key pro-inflammatory mediators. Binding to S100B (by similar) may contribute to myocyte apoptosis after myocardial infarction by activating ERK1/2 and p53/TP53 signaling. RAGE also facilitates the translocation of amyloid-beta peptide (ABPP) across neuronal membranes and mediates endothelial albumin transcytosis alongside HMGB1 via the RAGE/SRC/Caveolin-1 pathway, resulting in endothelial hyperpermeability. Additionally, RAGE loads HMGB1 into extracellular vesicles (EVs), which shuttle HMGB1 to hepatocytes via transferrin-mediated endocytosis, promoting hepatocyte pyroptosis through NLRP3 inflammasome activation. RAGE binds DNA and enhances cellular uptake of hypomethylated CpG DNA via endosomal routes to trigger inflammatory responses. In non-professional phagocytes (NPP), RAGE mediates phagocytosis, which is amplified by ligands like RNA, DNA, HMGB1, and histones. It also promotes NPP-mediated phagocytosis of Saccharomyces cerevisiae spores (via spore-wall RNA) and apoptotic cells. Following DNA damage, RAGE localizes to double-strand break sites, colocalizing with the MRN repair complex through interaction with MRE11 (by similar), and enhances MRE11's endonuclease activity to facilitate DNA end resection. RAGE further safeguards cells from replication stress by stabilizing the MCM2-7 complex and ensuring faithful replication fork progression. Moreover, RAGE mediates reactive oxygen species (ROS) production in human endothelial cells.
