Background

Lactoferrin is an iron-binding transport protein belonging to the transferrins family, capable of binding two Fe(3+) ions along with an anion, typically bicarbonate. It is a major iron-binding multifunctional protein found in exocrine fluids such as breast milk and mucosal secretions, exhibiting antimicrobial activity dependent on extracellular cation concentration. Its antimicrobial properties include bacteriostasis by sequestering free iron to inhibit microbial growth, as well as direct bactericidal effects causing the release of lipopolysaccharides from bacterial outer membranes. It also inhibits biofilm formation in Pseudomonas aeruginosa infections and demonstrates weak antifungal activity against Candida albicans. Lactoferrin exerts anabolic, differentiating, and anti-apoptotic effects on osteoblasts while inhibiting osteoclastogenesis, potentially regulating bone growth. It promotes species C adenovirus binding to epithelial cells, enhancing adenovirus infection, but inhibits papillomavirus infections. Additionally, it stimulates the TLR4 signaling pathway, leading to NF-kappa-B activation and pro-inflammatory cytokine production, while interfering with LPS-stimulated TLR4 signaling. When secreted by apoptotic cells, it inhibits neutrophil migration to apoptosis sites and stimulates VEGFA-mediated endothelial cell migration and proliferation. Lactoferrin binds heparin, chondroitin sulfate, and other glycosaminoglycans (GAGs), as well as specifically interacting with pneumococcal surface protein A (PspA), the lipid A portion of bacterial LPS, lysozyme, and DNA. Its derivative, lactoferricin, binds bacterial surfaces and plays a crucial role in bactericidal functions, exhibiting some antiviral activity against papillomavirus. The N-terminal region shows strong antifungal activity against C. albicans and contains two BBXB heparin-binding consensus sequences forming the primary GAG-binding site. Lactoferrin also displays opioid antagonist activity, with preference for μ or κ receptors (by similar). Its transferrin-like domain 1 functions as a serine protease of the peptidase S60 family, cleaving arginine-rich regions to enhance antimicrobial activity, preferentially targeting specific sequences (e.g., -Arg-Ser-Arg-Arg-|- and -Arg-Arg-Ser-Arg-|-). Furthermore, lactoferrin acts as a transcription factor with antiproliferative properties, inducing cell cycle arrest and binding to the DeltaLf response element in promoters of genes like SKP1 and BAX.
